If calling for an appointment, please have your insurance card available for reference.  If you are unable to keep your appointment, please notify us in advance, so that we can use the available time to care for another patient's needs.  Failure to appear for your appointment may result in a charge of $25.00.

Our office hours are 8:15 am to 4:15 pm.
